Client Care Coordinator

We are seeking a highly skilled and motivated Client Care Coordinator to oversee the case management of our Community Aged Care clients.
This role involves assessing, coordinating, and reviewing client care needs within budget constraints, ensuring delivery of flexible and responsive services tailored to individual requirements.
The ideal candidate will have a genuine passion for care, balanced with a desire to maximise services and offer commercially sustainable case management in line with organisational strategy.
- Proven experience in case management, including budgeting and financial planning.
- Experience in managing Home Care Packages or transferable case management skills in another field.
- Highly developed planning, influencing, and negotiation skills.
- Ability to manage contractual requirements, ensuring compliance with regulatory standards and organisational policy.
- Strong computer literacy and ability to learn new software platforms.
- Current drivers' licence and registered vehicle.
