Support Practitioner Role

We are seeking a dedicated Support Practitioner to play a critical role in improving the quality of life and developing new skills for individuals with challenging behaviours.

Key responsibilities include:

  • Assisting individuals with disabilities, their families, and/or carers to manage behaviour triggers, identify risks, and develop strategies for improved outcomes.
  • Conducting thorough assessments to understand behaviour triggers, identifying risks, and considering specific strategies to enhance behaviour outcomes.
  • Developing Support Plans and providing guidance to families and/or carers for consistent implementation.
  • Maintaining a caseload with ongoing monitoring, support, and adjustments where required.

As a valued member of our multidisciplinary team, you will collaborate with others to ensure each individual receives the best possible support.

Essential Qualifications

Requirements:

  • Extensive experience working within the disability sector, supporting individuals with challenging behaviours.
  • Qualified professionals or those with extensive experience in the disability sector across all age groups.
  • Passion for supporting individuals with complex needs and challenging behaviours.
  • Commitment to person-centred practice and reducing restrictive interventions.
  • Strong report writing skills and ability to translate assessment findings into clear, actionable plans.
  • Excellent time management and organisational skills to balance a dynamic caseload.
