Job Overview

In this pivotal role, you will empower individuals with unique needs to achieve their goals by crafting and implementing personalized plans.

This dynamic position involves a blend of in-person interactions, training sessions, and remote work. A driver's license, reliable transportation, and adequate insurance are necessary for out-of-town visits.

All full-time positions come with comprehensive benefits, including health, vision, and dental insurance, paid time off, $3000 tuition reimbursement per calendar year, and up to $250 of professional development courses. We also qualify for the Public Service Loan Forgiveness program.

Key Responsibilities
  • Advocate for and with members to ensure informed decision making, consent, and guardianship.
  • Schedule, lead, and collaborate with interdisciplinary teams to develop comprehensive, person-centered Life Plans that reflect individuals' needs and desired life goals.
  • Implement, update, and monitor Life Plans, facilitating individualized reviews and approval processes.
  • Ensure integration of necessary supports and services, including medical, behavioral, social, habilitation, dental, psychosocial, community-based, and facility-based long-term supports and services.
  • Maintain ongoing contact with critical people in members' lives.
  • Submit timely documentation (Life Plan, Progress notes, etc.).
  • Assist members with maintaining benefits, resolving problems in living, and reporting abuse or neglect when observed or reported.
Requirements

Candidates must meet the following qualifications:

  • A Bachelor's degree with 2+ years of relevant experience, OR a License as a Registered Nurse with 2+ years of relevant experience, OR a Master's degree with 1+ year of relevant experience.
  • Strong communication, interpersonal, and organizational skills are essential.
