Job Description

We are seeking a dedicated and compassionate Personal Care Worker to join our team. As a key member of our multidisciplinary team, you will be responsible for providing high-quality, person-centered care to residents in accordance with their individual needs and preferences.

Working in a fast-paced environment, you will provide effective clinical and emotional support to residents, recognize changes in their conditions, and escalate observations and concerns to the relevant authorities.

  • Deliver high-quality, person-centered care to residents and families, including consideration for cultural, spiritual, and individual needs.
  • Provide personal care to residents with complex needs such as palliative care, dementia support, restorative care, and pain management to enhance their wellbeing.
  • Support nursing staff regarding assessment, care planning processes, documentation, and evaluation of care outcomes.
  • Monitor the cleanliness, safety, environment, and promptly report any issues of concern.
Required Skills and Qualifications

To be successful in this role, you will require:

  • A Certificate III in Individual Support or equivalent (Aged Care).
  • Experience in residential aged care/home care is desirable.
  • Experience with dementia care and palliative care support is also desirable.
  • Excellent communication skills and the ability to work effectively in a team.
