About the Role

We are seeking an experienced and compassionate Disability Support Worker to join our team at Centacare. As a Disability Support Worker, you will play a vital role in providing high-quality care and support to individuals with disabilities, enabling them to live fulfilling and independent lives.

Key Responsibilities
  • Provide direct care services to clients, including personal care, daily living activities, and social support.
  • Develop and implement individualized support plans to meet the unique needs and goals of each client.
  • Facilitate community access and social events to promote socialization and skill development.
  • Maintain accurate documentation and records to monitor client progress and health care.
  • Collaborate with multidisciplinary teams to ensure comprehensive and person-centered care.
Requirements
  • Certificate III in Disability Services or Individual Support (or equivalent)
  • Valid Working with Children Check (WWCC) and NDIS Worker Check
  • Current First Aid and CPR certification
  • Ability to work outside of normal hours, including weekends and evenings
  • Valid Australian Driver's Licence
