About the Role
Summary

We are seeking an experienced Disability Support Worker to join our team at Centacare Adelaide metro. As a Disability Support Worker, you will provide high-quality direct care services to our clients and support them in achieving their personal goals.



Key Responsibilities

  • Supervise and support clients with daily living activities and personal care needs, either individually or as part of a team.
  • Facilitate community access opportunities for clients, including social events and medical appointments.
  • Maintain accurate documentation to monitor and report on client progress and health care.
  • Implement educational, skill development, and social activity programs for clients.
  • Assist in developing, implementing, and reviewing clients' Individual Support Plans to ensure personalized care.
  • Provide person-centred support tailored to each client's goals and foster increased skill development and independence.


About You

We are looking for an experienced support worker dedicated to helping people with disabilities. You will bring a comprehensive understanding of the NDIS Practice Standards and demonstrate a strong commitment to a person-centred approach. Your genuine passion for empowering individuals with disabilities will shine through as you assist them in achieving their goals and fostering the development of new skills.



Requirements

  • Certificate III in Disability Services or Individual Support or acceptable alternative.
  • A commitment to undertake a Certificate III in Individual Support within 6 months of starting, and to complete it within 2 years of employment.
  • Valid (DHS) Working with children check (WWCC) - employers check.
  • Valid NDIS Worker Check.
  • Valid Australian Driver's Licence.
  • Current First Aid and CPR certification.
  • Ability to comply with COVID-19 vaccination requirements.
  • Completion of the NDIS Quality and Safeguards Commission - New Worker NDIS Induction Modules and Worker Orientation Module.
  • Ability and availability to work outside of normal hours, including weekends, overnights and evenings.
  • Must be willing to work extra shifts, as required.
  • Willingness to transport clients either in a Centacare Vehicle or your own motor vehicle and comply with the Grey Fleet Vehicle Contract.


Preferred Qualifications

  • Experience working with people with a disability and their families.
  • Experience working in community based human service.
