We are seeking a passionate professional to provide quality support coordination, customer-focused assessment, and service delivery to ensure residents achieve their care needs.

The successful candidate will work collaboratively with residents, staff, internal and external providers, and health professionals to facilitate ongoing assessment, coordination, documentation, monitoring, review, and advocacy as required.

This rewarding role involves providing quality case-management services to residents, including liaison with NDIS, Support Coordinators, Plan Managers, to facilitate access to core supports, compliance reporting, and capacity building funding.

The ideal candidate will have excellent knowledge of the NDIS and exceptional customer service skills to work with residents, families, carers, advocates, and external agencies to ensure residents receive customised services and supports.

Key responsibilities include:

  • Delivering high-quality case management services to NDIS aged care residents
  • Providing support coordination and customer-focused assessment
  • Liaising with NDIS, Support Coordinators, Plan Managers, and other stakeholders
  • Monitoring and reviewing resident care plans to ensure optimal outcomes
