About the role

In this rewarding role you will be responsible for working with family-based placements where a child or young person is identified as having high complex to extreme needs and the placement requires additional support.

Your daily duties will be varied and challenging, but tasks include:

- Monitoring and assessing carer's ongoing suitability to continue as carers within the service;
- Supporting carers with placements and developing foster carer support plans;
- Accepting referrals for children and young people to be matched with carers in accordance with the service policy and through negotiation with the relevant government department;
- Developing and monitoring individual care plans for young people placed with specialist foster carers.

Well-developed communication skills are part of your skill set as you liaise comfortably with a range of internal and external stakeholders.

As an excellent multi-tasker, you are great at juggling priorities without compromising on quality. You have comprehensive knowledge of legislative and service requirements and readily foster a culture of compliance.

To be suitable for this position, you will have:

- A recognised Degree in Social Work, Psychology, Social Sciences or a related discipline;
- Experience working with Foster Carers supporting Complex to Extreme needs children is highly desirable as well as a solid understanding of NDIS;
- High level of case management, care coordination and/or complex case coordination;
- Highly developed adaptable and person-centred communication skills;
- Strong connections with disability, mainstream and community networks;
- Demonstrated experience in providing training to adults, undertaking assessments, and providing a range of supports in a child protection environment;
- Experience working with children and young people with disabilities (physical and cognitive) and/or complex challenging, sometimes extreme, behaviours is desirable but not essential;
- A current, or the ability to obtain a National Police Check, Licensed Care Service (LCS) check and a Working with Children (Blue Card);
- Up-to-date Covid-19 vaccinations;
- Current driver's licence.
