**Case Management Professional Role**

The Salvation Army is seeking a dedicated Case Management Professional to provide support in a community setting.

The ideal candidate will have strong interpersonal skills and the ability to establish relationships with individuals that encourage self-determination and participation.

The role involves:

  • Providing case management support to individuals, incorporating resilience, trauma-informed, and strengths-based practices.
  • Ongoing assessment of individuals' needs and goals from initial intake to exit.
  • Developing individual case plans in collaboration with the individual, addressing their needs or goals and barriers to achieving those.
  • Assisting individuals to access support services and establish community supports, providing advocacy where needed.

This role requires:

  • Strong communication and interpersonal skills.
  • Ability to work effectively with others to provide a comprehensive service.
  • Maintenance of accurate records and data in accordance with TSA standards and procedures.
