Job Title: Nurse Navigator

Cancer care requires a skilled professional to guide patients through the complex healthcare system. As a Nurse Navigator, you will play a vital role in ensuring that patients receive timely access to quality care.

Responsibilities:

  • Coordinate patient care throughout the continuum of cancer experience.
  • Identify gaps in care delivery and seek opportunities for improvement.
  • Lead Patient Care Conferences (huddles) to communicate interventional outcomes.
  • Assess patient risk stratification and develop nursing diagnoses.
  • Provide emotional support and make referrals as needed.
  • Support oral chemotherapy education and side effect monitoring.
  • Hold survivorship and advance care planning visits.
  • Participate in research, quality, and performance improvement.
  • Support patient advocacy and empowerment.

Qualifications:

  • Bachelor of Science in Nursing or related degree required.
  • Five years of work experience as a Registered Nurse in an oncology practice or clinic.

What You Bring to the Team:

  • OCN Certificate preferred.
  • Knowledge of healthcare systems and operations guidelines.
  • Ability to meet patient needs considering health literacy, culture, and language.
