Full-time Case Manager

This is a fixed-term opportunity for a skilled and experienced Case Manager to provide critical support services to adults and families at risk of, or experiencing, homelessness within the Bunbury community.

The successful candidate will be responsible for delivering high-level case management practices and strategies to support vulnerable individuals and families in securing and maintaining stable accommodation.

Key responsibilities include:

  • Developing and implementing effective case plans that address the unique needs of clients.
  • Establishing and maintaining collaborative relationships with external services, government agencies, and community groups to ensure comprehensive support.
  • Actively contributing to a positive team culture that prioritizes client-centred approaches and solution-focused outcomes.

Essential Qualifications:

  • Tertiary qualification in Social Work, Psychology, or a related field of study.
  • Proven experience working in homelessness and/or community services.
  • Demonstrated ability to deliver high-quality case management practices and strategies to support vulnerable populations.

Desirable Requirements:

  • Local knowledge of the Bunbury community and its challenges.
