**About the role**

Activ's transition home in Withers provides short-term support for those living with disability and requiring psychosocial support as they transition from a hospital stay to their future permanent home. The transition home is an incredibly rewarding place to be a part of and vacancies for new staff rarely become available.

We currently have a permanent night awake shift available. Night awake shifts require you to stay awake in the home overnight working alongside another support worker. Shifts run from evening until early morning with 7 shifts a fortnight including one weekend.

**A typical day as a Support Worker may look like**:

- Delivering a wide range of domestic supports within high-needs customer residence;
- Supporting person-centred plans that identify a customer's life goals;
- Ensuring positive behaviour support plans are followed; and
- Administration duties, expense and mandatory reports.
- Be qualified in Cert III in Individual Support (CHC33015 or CHC33021) - **essential**:

- As a high intensity home, experience with the following would be beneficial: supporting behaviours of concern, bowel care, catheter, diabetes, epilepsy, mental health and psychosocial support
- Willingness to support customers of any gender by assisting with personal care (showering, toileting, medication administration) and domestic duties (meal preparation, housekeeping)
- Passion for providing service and assistance and helping others live their best lives
- Hold a Cert IV in Mental Health (CHC43315) - well regarded, but not essential
- Willingness to use your fully insured and registered, private vehicle (aligned with Activ's motor vehicle allowance rebate).
