Job Title: Case Manager

Anglicare South, West & ACT is seeking a skilled Case Manager to join our Family Capacity Building Program in Eurobodalla. The successful candidate will be responsible for delivering client-focused and outcome-based services to vulnerable families.

  • Key Responsibilities:
  • Deliver high-quality case management services to clients with complex needs.
  • Work collaboratively with clients, families, and other service providers to achieve positive outcomes.
  • Develop and implement case plans that are tailored to the individual needs of each client.
Requirements:
  • Relevant qualifications in the Human Services field (Diploma of Community Services or equivalent; Degree qualifications preferred).
  • Practical experience in working with vulnerable families and an ability to identify children and young people at risk.
  • Demonstrated expertise in client-centred case management.
Benefits:
  • Salary range from $44.58 to $47.97 per hour, plus 12% superannuation.
