This Position is Part of Our APM Futures Program

We are seeking a motivated and caring individual to join our team as a facilitator, working within our APM Futures program based in Werribee. The role commences at 4 days per week, 8.30am - 5.00pm (hours could increase depending on caseload growth).

About the Role

Our APM Futures program delivers highly individualised employment supports to NDIS Participants, helping them develop and establish their vocational identity.

Your key responsibilities will include:

  • Providing hands-on & highly consultative support to a small group of participants
  • Mentoring, encouraging skills development and increasing people's capacity for work
  • Delivering person-centred supports to participants, some transitioning from school to work
  • Providing on-site support once employment has commenced and assisting with integration
  • Supporting and consulting employers

Qualifications and Requirements

We welcome applications from those with experience in Youth Services, NDIS, Community Services and Employment Services. You should be a resilient and creative problem solver who thrives in a collaborative environment.

To be successful in this role, you will need:

  • Eligibility to work in Australia
  • A current driver's licence and comprehensively insured vehicle
  • Current First Aid and CPR certificate
  • Ability to obtain a Working with Children Check & NDIS Worker Screening Check
  • Ability to successfully complete a NDIS Worker Orientation Module prior to commencement
  • Travel as required for service delivery and support of participants
