CatholicCare Diocese of Broken Bay is seeking a skilled and compassionate Case Manager to join our Foster Care services in Sydney or on the Central Coast. As a Case Manager, you will work with young people and their care teams to deliver case management across a range of therapeutic services.
Key responsibilities include:
- Working with foster and kinship carers to provide holistic and therapeutic day-to-day care of children and young people.
- Case planning and holistic assessment and support of families, children and young people to promote and support their safety, well-being and permanency needs.
- Developing pathways for young people to transition through care and ultimately leave care through an appropriate permanency pathway.
- Timely completion of administration tasks and case notes.
To be successful in this role, you will have:
- A Degree qualification in a relevant field and ideally have additional training or qualifications in developmental trauma, child protection, client intervention and case management.
- Demonstrated case work experience working with children and young people who have experienced complex trauma.
- Experience working with families who have been in contact with the child protection system.
- Exceptional written and oral communication with the ability to build relationships.
