Job Description:

The role of a Community Nurse involves providing high-quality, evidence-based care in wound management, catheter and drain care, and palliative support. Our team works autonomously and collaboratively, with a strong focus on education, support, and clinical excellence.

Key Responsibilities:
  • Provide home visits throughout the community to provide nursing care.
  • Work collaboratively with clinicians in our Community Nursing team at Wyong Central Community Health Centre.
  • Develop responsive strategies to adapt to the changing health needs of the community.
Required Skills and Qualifications:
  • You have a current registration as a Registered Nurse (Division 1) with AHPRA.
  • You have recent nursing experience in Community Nursing or a demonstrated interest in Community Nursing.
  • You have demonstrated ability to liaise with other health professionals to facilitate case management and care planning.
