Deliver exceptional client care by coordinating complex needs and collaborating with multidisciplinary teams.
- Develop and review comprehensive care plans in partnership with clients and healthcare professionals.
- Manage resources and facilitate collaborative care activities.
- Liaise with clinical teams to monitor and respond to changing client needs.
- Continuously evaluate and refine care plans, services, and outcomes.
You will spend most of your time on the road, engaging with clients in their communities.
Key Responsibilities:- Hold a current registration as a Registered Nurse or Allied Health professional and have unlimited work entitlements.
- Demonstrate expertise in delivering community-based care services and knowledge of relevant funding streams.
- Showcase excellent communication and problem-solving skills.
- Possess a current Australian Driver's Licence and a roadworthy vehicle for work purposes.
- Exhibit confidence in managing time effectively and working independently.
