Community Home Care Provider

We are seeking a compassionate and skilled Home Care Specialist to join our Community Care Team. In this rewarding role, you will provide personal care and support to clients in their own homes, enabling them to live independently and maintain their quality of life.
- Provide high-quality in-home care and assistance with daily living activities, including personal hygiene, meal preparation, house cleaning, and laundry
- Support clients with community-based recreational and social activities, promoting their physical and emotional well-being
- Transportation and assistance with shopping and appointments, ensuring seamless coordination of client care
- Deliver care in accordance with individualized Care Plans, tailoring your approach to meet the unique needs of each client
To succeed in this role, you will require a Certificate III Aged Care or Individual Support qualification, as well as experience working with technology, including smartphones, tablets, and laptops. You must also be able to satisfy relevant probity checks required by legislation or policies.
Key Responsibilities:- Assist clients with daily tasks and activities, promoting their independence and dignity
- Develop and implement individualized Care Plans, working closely with clients, families, and healthcare professionals
- Provide education and support to clients and their families on healthy aging and disease prevention
- Collaborate with multidisciplinary teams to ensure comprehensive care and support for clients
- Certificate III Aged Care or Individual Support (essential)
- Current Australian driver's license and own roadworthy insured vehicle (essential)
- Experience working with technology, including smartphones, tablets, and laptops
- Ability to satisfy relevant probity checks required by legislation or policies
