Vulnerable Children Case Manager

This highly rewarding position involves working closely with carers and children to create meaningful, lasting change.
Key Responsibilities:- Develop and implement individualised plans to achieve therapeutic goals through intensive work with carers and children.
- Collaborate with a small, close-knit team to provide comprehensive support to young people and their families.
- Build and maintain strong relationships with carers, foster families, and other stakeholders to ensure continuity of care.
- Deliver ongoing training and education to enhance the skills and knowledge of carers and foster families in supporting vulnerable children.
- Tertiary qualification in Psychology, Social Work, or a related discipline.
- Training and experience in child, adolescent, and family practice, case management, and trauma-informed care.
- Demonstrated understanding of Attachment and Trauma theories, intellectual disability, and ability to assist others to understand the implications.
- Extensive experience working with vulnerable children, young people, and families in a trauma-informed way.
