Job Opportunity

We are seeking a professional to join our home care team in the Central Coast. The successful candidate will be responsible for delivering high-quality clinical care and support services to participants.

The role involves working as a clinical partner in relation to the provision of home, centre and community-based supports and packages. Ensuring the highest quality of care is provided to participants is paramount.

  • Develop and implement clinical and general care plans based on individual participant needs and goals.
  • Manage workload effectively to achieve program performance goals and ensure timely delivery of services.

Key responsibilities include:

  • Functioning as a clinical partner in relation to the provision of home, centre and community-based supports and packages.
  • Ensuring the highest quality of care is provided to Anglican Care participants.

The ideal candidate will possess strong knowledge and practical experience working with people who are ageing from an enablement, restorative and wellbeing perspective.

Requirements
  • Current registration under the Australian Health Practitioner Regulation Agency (AHPRA).
  • Unrestricted drivers' licence.
  • NDIS Worker screening clearance.
  • CPR Certificate (annual certification).
  • National Police Check.
  • Australian work rights.
