Job Title

Clinical Care Specialist

As a Clinical Care Specialist, you will play a vital role in delivering high-quality, person-centered clinical care in the community setting. You will work collaboratively with our team to achieve best outcomes and provide guidance to care staff on safe delivery of clinical care.

Key Responsibilities:
  • Assess clinical care needs and develop comprehensive care plans
  • Deliver evidence-based clinical care to individuals
  • Collaborate with care management teams to ensure seamless service delivery
  • Provide expert guidance to care staff on safe delivery of clinical care
Required Skills and Qualifications:

To succeed in this role, you must possess a nursing qualification and be currently registered with AHPRA as a Registered Nurse (Division 1, General). You should also have a passion for client-focused service delivery in aged and community/home care settings, as well as knowledge of Aged and Home Care Reforms and Aged Care Quality Standards.

In addition, you should have excellent communication and collaboration skills, with the ability to identify and implement innovations in care delivery and service improvement.
