Aged Care is a vital sector that requires dedicated professionals to ensure older adults receive the care and support they need to live independently. In this role, you will play a crucial part in assessing the needs of seniors and connecting them with essential services.

Your Key Responsibilities:

  • Conduct thorough assessments to identify the needs of older adults.
  • Facilitate referrals to services that support independent living.
  • Promote functionality and capability in daily living.
  • Engage with families and carers as key stakeholders.
  • Ensure assessments are conducted in line with contractual requirements.
About You

We are seeking individuals with at least 1 year of experience in aged care, including client assessments and/or service delivery. A tertiary education in a health-related discipline, such as nursing, occupational therapy, physiotherapy, or social work, is also required. Additionally, you must have current unrestricted General Registration with AHPRA or a relevant professional association.

You should be proficient in using Word, Excel, Outlook, and client management systems. A current Australian driver's licence, comprehensively insured vehicle, and ability to travel within your service area are also necessary.
