About This Role

This Aged Care Clinical Assessor role is a challenging and rewarding position where you will play a crucial part in identifying the needs of older adults and connecting them with services that help them live independently with the highest quality of life.

You will be conducting thorough Aged Care Assessments to identify the needs of older adults, facilitating referrals to essential services that support independent living, actively promoting the retention and maximization of function and capability in daily living, working closely with families and carers, recognizing them as key stakeholders, and ensuring all assessments are conducted in line with Department of Health contractual requirements.

Responsibilities
  • Conduct high-quality assessments to identify the needs of older adults.
  • Facilitate service referrals to support independent living.
  • Promote functionality to maximize the capabilities of daily living.
  • Engage stakeholders by working closely with families and carers.
  • Evaluate compliance with Department of Health contractual requirements.
About You

To succeed in this role, you will need at least 1 year of experience in aged care, including client assessments and/or service delivery, a tertiary education in a health-related discipline, current unrestricted General Registration with AHPRA or relevant professional association, proficiency in Word, Excel, Outlook, and client management systems, and a current Australian driver's license.
