Job Description:

As a skilled Aged Care Clinical Assessor, you will play a vital role in identifying the needs of older adults and connecting them with essential services that support their independence and overall well-being.

Our team works collaboratively to deliver high-quality care, ensuring that every individual receives the attention and support they require. You will be responsible for conducting thorough assessments, facilitating referrals, and promoting the retention and maximization of function and capability in daily living.

Key Responsibilities:

  • Conduct comprehensive aged care assessments to identify the needs of older adults.
  • Develop and implement effective care plans that address the unique requirements of each individual.
  • Collaborate with families, carers, and other healthcare professionals to ensure seamless care delivery.
  • Ensure all assessments are conducted in accordance with relevant contractual requirements.

Required Skills and Qualifications:

  • At least 1 year of experience in aged care, including client assessments and service delivery.
  • Tertiary education in a health-related discipline or equivalent qualification.
  • Current unrestricted General Registration with AHPRA or relevant professional association.
  • Proficiency in Microsoft Office applications, including Word, Excel, and Outlook.
  • Current Australian driver's licence and comprehensively insured vehicle.
