Aged Care Clinical Assessor Job Summary
We are seeking an experienced Aged Care Clinical Assessor to join our team. As a vital member of our team, you will play a crucial role in identifying the needs of older adults and connecting them with essential services to support independent living.

Your Key Responsibilities:
  • Conduct thorough assessments to accurately identify the needs of older adults.
  • Facilitate referrals to services that promote independent living, functionality, and capability in daily life.
  • Work closely with families and carers, recognizing their importance as key stakeholders.
  • E nsure all assessments meet Department of Health contractual requirements.

Requirements:
  • At least 1 year's experience in aged care, including client assessments and/or service delivery.
  • Tertiary education in a health-related discipline (e.g., nursing, occupational therapy, physiotherapy, social work).
  • Current unrestricted General Registration with AHPRA or relevant professional association.
  • Proficiency in Word, Excel, Outlook, and client management systems.
  • A current Australian driver's licence, comprehensively insured vehicle, and ability to travel within your service area.
