About the Role

As a Clinical Pharmacist, you will have the opportunity to contribute to the health and wellbeing of our community by providing high-quality clinical pharmacy services to patients and healthcare teams.

This role involves working across multiple campuses, providing medication history documentation, reconciling prescribed medications, managing medication supply and preparation, as well as providing education and support to patients and their carers.

Responsibilities
  • Compile and document best possible medication history for hospital inpatients
  • Reconcile and clinically review prescribed medications
  • Manage supply of inpatient and discharge medications for hospital patients and some outpatients
  • Check prepared medications prior to delivery to wards and patients
  • Prepare discharge medication and discharge medication summaries
  • Provide medication education and counselling for patients and their carers
  • Supervise and assist a team of pharmacists, technicians and students; contribute to improvement activities
  • Liaise with external facilities and community healthcare providers to ensure continuity of care
  • Provide medication-related information, advice and support to the multidisciplinary team
