Healthcare Social Work Specialist

The primary objective of this position is to deliver high-quality clinical social work services to inpatients at an acute medicine department within a hospital. This entails conducting thorough assessments, developing and implementing care plans, and providing interventions to meet the psychosocial needs of patients.

This role requires collaboration with various healthcare teams and community providers to ensure seamless patient care and optimal health outcomes. The ideal candidate will possess excellent communication and interpersonal skills, as well as the ability to work independently and make professional decisions.

Key Responsibilities:
