About the Care Partner Role

As a key point of contact, you will provide support to the broader community team and work closely with clients and their families.

">

To be successful in this role, you must hold a formal tertiary qualification such as a Bachelor of Nursing or equivalent, current AHPRA registration as a Registered Nurse, national criminal history/police check, valid First Aid and CPR certificate, and a valid Working With Children's Check Card.

Experience in the aged care industry is essential, with knowledge of issues such as Continuous Quality Improvement, the Aged Care Quality Principles, and the Aged Care Quality Standards. Previous experience in Aged Care or a related discipline is desirable, as well as being easily adaptable to new software platforms.

Key responsibilities include building strong connections with clients and their families, conducting comprehensive client assessments and developing care plans, interacting with home care workers, providing professional health and clinical care advice, professionally liaising with clients and their families, ensuring all practices are consistent within policy and legislative frameworks, and maintaining up-to-date client records and information.
