We’re currently seeking a passionate and experienced Care Partner to help deliver high-quality in-home and community-based care. If you’re someone who’s caring, dependable, and values meaningful work—we’d love to hear from you.
**What You'll Do**
- Provide personalised support that helps people remain safe and independent at home
- Conduct client assessments and develop tailored care plans
- Deliver care services such as medication prompts, continence support, chronic disease monitoring, and general well-being checks
- Supervise support staff and coordinate care delivery in line with funding and service plans
- Complete reports and administrative tasks relating to care
**About You**
- Experience in aged care, disability support, or community care
- Excellent communication skills and a client-first approach
- Confident working both independently and within a team
- Great time management and attention to detail
- Strong values around dignity, respect, and empowerment
- Bonus: If you’re a Registered or Enrolled Nurse, that’s a big plus—but not essential_
**Requirements**:
- First Aid & CPR certification
- National Police Check
- Valid Driver’s Licence and reliable, presentable vehicle
- NSW Working with Children Check
- Willingness to obtain a NDIS Worker Screening Check (if not already held)
