We are seeking an experienced Geriatric Care Specialist to join our dedicated Aged Care team.

The successful candidate will coordinate and provide professional nursing care, adhering to policy, procedures, and best practice guidelines while maintaining the highest standards of privacy and confidentiality.

Key Responsibilities:
  • Deliver patient-centered care, consistently meeting or exceeding expectations and following established protocols.
  • In consultation with the Program Coordinator, develop Behaviour Support Plans based on sound dementia care principles, ensuring client well-being and satisfaction.
  • Collaborate with the Program Coordinator to complete, review, and action Care Plans and clinical assessments as required, contributing to continuous quality improvement.
  • Promote client rights and interests by adhering to relevant reporting mechanisms and within defined limits of responsibility.
  • Manage and report incidents, accidents, or untoward events according to established policies and procedures.
Requirements:
  • Current registration as a Registered Nurse with AHPRA and medication endorsement.
  • Proven experience in delivering high-quality care to clients, including those with complex needs and diverse backgrounds.
  • Demonstrated ability to work independently and effectively as a team member to achieve planned outcomes.
  • Demonstrated knowledge and competency in Microsoft Office Programs, including Word, Excel, and Outlook.
