Job Description

As a Connected Care Service Development Manager, you will play a key role in leading the development and implementation of innovation projects that align with the Transforming Your Experience (TYE) strategy. You will evaluate current services, identify opportunities for innovation, develop new virtual clinical pathways, and lead change management initiatives.

You will work closely with clinical and non-clinical staff, patients, and consumers to ensure that SWSLHD services meet community needs. This includes developing and implementing remote patient monitoring services across the district.

Key Responsibilities:
  • Evaluate current services and identify opportunities for innovation.
  • Develop new virtual clinical pathways.
  • Lead change management initiatives.
