As a senior caregiver, you'll conduct initial client intake and screening assessments and develop and review individualized care plans while providing quality care based on clients' needs.
- Conduct thorough initial screening assessments and develop care plans for personal self-care, general house cleaning, social support, respite, transport, and shopping assistance.
- Manage client information in CRM & CMS ensuring all details and case notes are up to date.
- Engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes in consultation with the care team.
- Spend time on the road completing scheduled visits, interacting one-on-one with clients within their homes, supporting daily activities.
- Provide care in accordance with individualized Care Plans and support community-based recreational and social activities.
- Transportation assistance with shopping and appointments.
- Certificate III Aged Care, Individual Support or equivalent qualification – Essential
- Current Australian Driver's Licence (Green Provisional minimum) – Essential
- Roadworthy & insured (Third Party Property minimum) vehicle for work – Essential
- Experience with technology (smartphones, tablets, laptops, etc.)
- Ability to meet probity checks required by legislation or policy
