Job Summary

The Aged Care Coordinator will manage the service for new Commonwealth Home Support Program (CHSP) clients. The role involves providing excellent customer service to clients, families, and stakeholders, ensuring all inquiries are addressed promptly and professionally.

The Coordinator will assist with the intake of clients and scheduling of services, and take on a leadership role to deliver quality Home Care services. This includes guiding, mentoring, and training staff, providing learning opportunities and development, and supporting staff to deliver quality care that promotes client choice and independence.

Key Responsibilities
  • Manage the service for new CHSP clients
  • Provide exceptional customer service to clients, families, and stakeholders
  • Assist with client intake and scheduling of services
  • Take on a leadership role to deliver quality Home Care services
  • Guide, mentor, and train staff
  • Provide learning opportunities and development for staff
Requirements

This is an ideal role for someone passionate about providing high-quality services to our clients. We are seeking an individual with fantastic coordination skills and ideally experience within the CHSP program. You will have the ability to communicate effectively and foster a great team environment.

The successful candidate will possess:

  • Cert IV in relevant industry qualification and/or significant industry experience
  • Current unencumbered Driver's Licence
  • Current first aid and CPR
  • Satisfactory Criminal Record Check
  • Capacity to interpret and work within relevant program guidelines
  • Ability to mentor, share knowledge and information with less experienced team members
  • Ability to manage a complex workload by planning and organisation of priorities
