Key Responsibilities
- Provide expert case management and care coordination, delivering person-led, strength-focused support to clients accessing our recovery service.
- Collaborate as part of a dedicated multidisciplinary team making a real impact in the local community.
- Work closely with clients to understand their needs and develop personalized support plans.
- Build and maintain relationships with community partners and stakeholders to ensure seamless service delivery.
- Contribute to the development and implementation of programs and services that address the needs of our community.
What We Offer
- SCHCADS Award LV5: hourly rates from $49 to $51.51 (dependent on experience)
- Generous NFP Packaging Benefits increasing total pay package up to 18k.
