Key Responsibilities

  1. Provide expert case management and care coordination, delivering person-led, strength-focused support to clients accessing our recovery service.
  2. Collaborate as part of a dedicated multidisciplinary team making a real impact in the local community.
  3. Work closely with clients to understand their needs and develop personalized support plans.
  4. Build and maintain relationships with community partners and stakeholders to ensure seamless service delivery.
  5. Contribute to the development and implementation of programs and services that address the needs of our community.

What We Offer

  1. SCHCADS Award LV5: hourly rates from $49 to $51.51 (dependent on experience)
  2. Generous NFP Packaging Benefits increasing total pay package up to 18k.
