In this dynamic role, you will utilize technology and contribute to shaping exceptional health outcomes for sub-acute patients. Your key responsibilities include:

  • Collaborating with a multidisciplinary team to develop and implement integrated service models and clinical protocols.
  • Facilitating group sessions and contributing to health promotion activities as required.
  • Providing effective assessment and triage, timely referrals, accurate information, coordinated care, and prompt follow-up.
  • Advocating for patients' needs and coordinating access to services.

About You

You are passionate about providing innovative models of care and excited by this dynamic way of working. Additionally, you have:

  • A degree in Social Work, with eligibility for membership with the Australian Association of Social Workers (AASW).
  • Demonstrated experience in a sub-acute or similar healthcare setting.
  • Strong communication, advocacy, and problem-solving skills.
  • Excellent communication and interpersonal skills, with the ability to adapt to evolving service needs.
  • Proficiency in using Microsoft Office suite and other relevant documentation systems.
  • A South Australian Working with Children Check and National Police Clearance (or willingness and eligibility to obtain).
  • The ability to provide proof of up-to-date vaccinations.
