We are seeking a full-time Comprehensive Care Coordinator to deliver comprehensive care for patients with varying complexity on a fixed-term contract.
In this dynamic role, you will utilize technology and contribute to shaping exceptional health outcomes for sub-acute patients. Your key responsibilities include:
- Collaborating with a multidisciplinary team to develop and implement integrated service models and clinical protocols.
- Facilitating group sessions and contributing to health promotion activities as required.
- Providing effective assessment and triage, timely referrals, accurate information, coordinated care, and prompt follow-up.
- Advocating for patients' needs and coordinating access to services.
- Participating actively in clinical review meetings and continuous professional development.
You are passionate about providing innovative models of care and excited by this dynamic way of working. Additionally, you have:
- A degree in Social Work, with eligibility for membership with the Australian Association of Social Workers (AASW).
- Demonstrated experience in a sub-acute or similar healthcare setting.
- Strong communication, advocacy, and problem-solving skills.
- Excellent communication and interpersonal skills, with the ability to adapt to evolving service needs.
- Proficiency in using Microsoft Office suite and other relevant documentation systems.
- A South Australian Working with Children Check and National Police Clearance (or willingness and eligibility to obtain).
- The ability to provide proof of up-to-date vaccinations.
