Comprehensive Social Work Services Role

We are seeking a skilled professional to provide comprehensive social work services to patients, their carers and families at Flinders Medical Centre.

The successful candidate will report to the Manager of Community Palliative Care and the Director of Social Work, working closely with other team members and community agencies to assess patients' needs and develop intervention programs.

The role involves:

  • Assessing patients' abilities, problems and goals using various assessments
  • Planning and implementing intervention programs
  • Collaborating with other team members and community agencies
  • Linking patients and carers with relevant community resources and services

To be successful in this role, you will have high levels of responsibility and organisational skills, including clinical knowledge and experience working with loss, grief and bereavement. A Degree or equivalent qualification is required, along with eligibility for full membership of the Australian Association of Social Workers.

Key Responsibilities:

  • Providing person-centered care and support to patients and their families
  • Developing and implementing individualized care plans
  • Collaborating with multidisciplinary teams to ensure holistic patient care
  • Maintaining accurate records and reports

Required Qualifications and Skills:

  • Relevant degree in social work or a related field
  • Eligibility for full membership of the Australian Association of Social Workers
  • Strong clinical knowledge and experience working with loss, grief and bereavement
  • Excellent communication and interpersonal skills
