Care Professionals Wanted

We are seeking dedicated professionals to join our team in providing person-centred care services.
This role is part of our From Hospital to Home (FH2H) service, which offers a more home-like environment than hospital with all necessary disability and social supports available.
The FH2H service supports people with disability for up to 18 weeks following their discharge from hospital.
As a Person-Centred Care Specialist, you will work closely with our Accommodation and Support Coordinator and be part of an inter-disciplinary team.
Your key responsibilities will include:
- Providing empathic support to consumers
- Assisting consumers in accessing relevant community services
- Engaging with external stakeholders regarding consumers' care plan development
- Delivering culturally informed support
To be successful in this role, you will hold:
- Cert III in Individual Support (Disability)
- National Police Clearance (obtained within last six months)
- NDIS Worker Screening Check (or be willing to obtain)
- Evidence of COVID-19 and Influenza vaccinations (or be willing to obtain)
