Care Coordinator Role
We are seeking a compassionate and dedicated individual to join our team as a Care Coordinator. This is an exciting opportunity for someone who is passionate about providing high-quality care and support to our residents.
Responsibilities:
- Provide proactive care that enriches residents' daily lives by identifying their needs and preferences, and developing personalized care plans that meet their unique requirements.
- Support residents' autonomy and independence by empowering them to make informed decisions about their care and facilitating access to resources and services that promote their well-being.
- Deliver personalized care that meets best practice standards by staying up-to-date with the latest developments in aged care and implementing evidence-based practices in your work.
Requirements:
- A genuine passion for caring for others and making a positive difference in people's lives.
- A Certificate III or higher in Individual Support, Nursing, or related health/care studies, or equivalent experience in a care coordination role.
- Prior experience as a personal carer in aged care is highly regarded, but we also welcome new graduates or individuals looking to transition into a care coordination role.
