Job Title: Patient Care Coordinator

About The Role:

This is a fantastic opportunity for an experienced healthcare professional to join our team and deliver high-quality patient care. As a Patient Care Coordinator, you will play a critical role in coordinating individualized care plans that meet the unique needs of each patient.

  • You will work closely with our multidisciplinary team to assess patient needs, develop comprehensive care plans, and implement evidence-based interventions.
  • Collaborate with patients, families, and healthcare providers to ensure seamless transitions and optimal outcomes.
  • Evaluate patient progress, revise care plans as needed, and communicate effectively with all stakeholders.
  • Demonstrate expertise in patient-centered care, prioritizing empathy, respect, and compassion in all interactions.
Required Skills and Qualifications:
  • A registered nurse with current AHPRA registration or equivalent qualification.
  • Minimum 2-3 years of clinical experience in a similar role, preferably with cardiac experience.
  • Demonstrated proficiency in interpersonal skills, effective communication, and teamwork.
  • Ability to work well under pressure, prioritize tasks, and manage competing demands.
