About this opportunity
We are seeking a skilled professional to fill the role of Case Manager.
You will develop and implement case plans for children and young people in care, working closely with young people, carers and their families to achieve permanency goals.
Your key responsibilities will include:
- Developing and implementing individualised case plans that meet the unique needs of each young person
- Providing ongoing support and guidance to carers and young people to help them achieve their objectives
- Maintaining effective relationships with carers, young people, families and other stakeholders
- Participating in regular team meetings and contributing to the development of the Permanency Support Program
To be successful in this role, you will need:
- A tertiary qualification in Social Work, Community Welfare, Psychology or a related field
- Demonstrated experience in case management and case planning
- Strong interpersonal and communication skills
- The ability to maintain confidentiality and handle sensitive information
