Job Title: Mental Health Outreach Specialist

We are seeking a highly skilled and compassionate mental health professional to provide support and guidance to individuals in their homes, promoting recovery and personal growth.

Key Responsibilities:

  • Develop and maintain strong relationships with patients and their families, ensuring effective communication and trust.
  • Work autonomously in a fast-paced environment, handling challenging situations and prioritizing tasks efficiently.
  • Collaborate with healthcare teams to deliver high-quality care and support services.
  • Maintain accurate records and reports, utilizing IT systems effectively.

Requirements:

  • Background in working with older adults with mental illness or similar experience.
  • Proven ability to engage patients in their recovery journey, fostering positive outcomes.
  • Excellent communication and interpersonal skills, with the capacity to work independently and as part of a team.
  • Proficiency in using Microsoft Office and other relevant software applications.
  • Valid driver's license and willingness to travel extensively.
  • Membership in the PVG Scheme and eligibility to work in the United Kingdom.

Working Conditions:

This is a full-time position, working regular office hours Monday to Friday. The role may require occasional weekend work and flexibility to accommodate changing patient needs.
