Job Title: Outreach Support For Children

An exciting opportunity exists to join our team providing intensive case management and interventions for young people who experience serious mental health issues. Working in a multidisciplinary setting with both young people and families to support their recovery and well-being.

  • We are seeking a qualified professional to provide systemic therapy and interventions for young children and adolescents with mental health problems, their families and systems of care and support.
  • The successful candidate will work closely with our team to develop and implement individualized plans that address the unique needs of each young person and family.

Key Responsibilities:

  1. Provide intensive case management and interventions for young people with complex mental health needs.
  2. Work collaboratively with families, carers and other professionals to support young people's recovery and well-being.
  3. Develop and implement individualized plans that address the unique needs of each young person and family.
  4. Maintain accurate and comprehensive records of client interactions and progress.

Requirements:

  • Recognised degree in Psychology, Social Work, Nursing or Occupational Therapy with registration with AHPRA or membership to AASW.
  • Demonstrated knowledge and skills in the assessment, formulation and treatment of child and adolescent mental health problems and family relationship problems.
  • Demonstrated capacity for multi-disciplinary teamwork and ability to contribute to team learning and development.
