About the Role

Mercy Community Disability Service is seeking a passionate and dedicated Support Coordinator to join our team in Gladstone. As a Support Coordinator, you will play a vital role in helping individuals with disabilities to understand and maximize their NDIS plans.

Key Responsibilities
  • Support participants to create and manage their NDIS plans, goals, and funding access
  • Coordinate and plan participants to access support services in accordance with their allocated NDIS funds/budgets
  • Connect participants with support services to pursue and achieve their goals
  • Use NDIS systems and processes to achieve the most of NDIS plans for participants
  • Monitor, support, track, and review participants' NDIS plans and goals
  • Complete and provide reports to the NDIA with outcomes and success indicators relating to participants' plans and goals
About You

To be successful in this role, you will possess:

  • Experience as a NDIS Support Coordinator (highly desired)
  • A current Queensland Blue Card (or willingness to apply)
  • A current NDIS Workers Screening Clearance (or willingness to apply)
  • A current QLD Drivers Licence and own vehicle (mandatory)
