Job Title: Support Advocate

The Support Advocate plays a vital role in assisting individuals to navigate their support plans, develop capacity to coordinate and manage their own services.

Primary Objectives:

  • Assist individuals to implement their support plans, increase capacity to manage/direct their own services
  • Have greater opportunities to explore and connect with their community and alternative service options
  • Strengthen informal support networks
  • Maximise the value of support received from services
  • Access and use online portals to manage support plans

Key Accountabilities:

  • Process referrals in a timely manner
  • Support individuals to assess mainstream, community, informal and provider options
  • Select preferred options or providers
  • Negotiate services to be provided and their prices
  • Develop service agreements and create service bookings with preferred providers
  • Arrange assessments required to determine funding requirements
  • Collaborate with service managers to establish claim categories and attribute correct amounts of funds
  • Link to mainstream or community services (housing, education, transport, health)
  • Strengthen and enhance individuals' capacity to coordinate services, self-direct and manage services and participate in the community

Working Relationships:

  • Develop strong relationships with customers, families, carers, advocates, service providers, community members, networking groups, government agencies, auditors and practitioners
  • Collaborate with Area Managers, Supported Living Managers, Individual Support Managers, Customer Service Officers and other Support Advocates
  • Share knowledge and experience with colleagues and members of the Customer Engagement team
  • Work with administrative staff

  • Ability to work with minimal supervision
  • Ability to build and maintain strong community connections and relationships with service providers, individuals and/or families/carers
