Clinical Nurse Specialist

We are seeking a highly skilled and dedicated Clinical Nurse Specialist to join our team at Integratedliving Australia. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and support to our clients in regional and remote areas.
Key Responsibilities- Provide direct client care through health, wellbeing, and clinical services based on established care plans.
- Work collaboratively with other healthcare professionals to assess and identify client needs and goals.
- Develop and implement service recommendations within defined funding parameters.
- Provide health promotion and education to clients and carers.
- Monitor health outcomes via telehealth and provide direct care services across various funding programs.
- Assess, develop, and evaluate nursing care for all clients through a diverse and collaborative approach.
- Support clients to become independent in managing their health needs through a wellness and reablement approach.
- Understanding of community nursing and primary health care principles, including providing services in a consumer-directed care framework.
- Community, acute, and aged care experience of 1 or more years.
- Experience working as an independent clinical practitioner or in a team.
- Ability to work systematically and accurately.
- Proficient in the use of electronic technology, including smartphones, tablets, laptops, email, internet, and client data management systems.
- Registered with the Australian Health Practitioners' Regulation Authority (AHPRA).
- Experience providing advanced clinical care, including chronic disease management, medication management, palliative care, catheter management, stoma care, wound management, and diabetes management.
- Established holistic assessment skills and proficient practical skills.
- Current driver's license and ability to travel for client care.
