Social Worker

We are seeking a highly skilled and compassionate Social Worker to join our team at Integratedliving Australia. As a Social Worker, you will play a vital role in promoting the wellbeing of our aged and vulnerable clients, enabling them to live independent lives in their own community.

• Conduct initial and ongoing assessments of clients' circumstances, making recommendations to resolve complex and unique situations.
• Provide direct client contact and clinical care, as well as advocacy work, both via telehealth and face-to-face interactions.
• Manage a portfolio of clients with diverse needs, including dementia, housing, access to services, guardianship, or elder abuse.
• Collaborate with a multidisciplinary team to provide holistic care to clients and liaise with their carers, case managers, and other health partners.
• Contribute to Wellness for Independence group programs and promote health and wellbeing in the community.

• Bachelor Degree or higher in Social Work and AASW membership.
• Satisfactory Police check.
• Current First Aid and CPR certification.
• Current driver's license and ability to travel for client care.
• Highly effective communication and interpersonal skills.
• Well-developed IT skills and experience with client management software.
• Experience working with a multidisciplinary team and exposure to collaborative practices.
• Sound clinical knowledge and ability to deal with complex issues.
