We are seeking a highly skilled and dedicated Clinical Nurse Specialist to join our team at Integratedliving Australia. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and support to our clients in regional and remote areas.
Key Responsibilities- Provide direct client care through health, wellbeing and/or clinical services based on established care plans.
- Work in collaboration with other healthcare professionals to assess and ascertain clients' needs and goals.
- Develop and implement service recommendations within defined funding parameters.
- Provide health promotion and health education to clients and carers.
- Monitor health outcomes via Telehealth.
- Provide direct care services across a range of different funding programs.
- Assess, develop and evaluate nursing care for all clients through a diverse and collaborative approach.
- Support clients to become independent with managing health needs through a Wellness and Reablement approach.
- Understanding of community nursing and the principles of primary health care.
- Community, acute and aged care experience of 1 or more years.
- Experience working as an independent clinical practitioner or in a team.
- Ability to work systematically and in an accurate manner.
- Proficient in the use of electronic technology.
- Registered with the Australian Health Practitioners' Regulation Authority (AHPRA).
- Experience providing advanced clinical care, including chronic disease management, medication management, palliative care, catheter management, stoma care, wound management and diabetes management.
- Established holistic assessment skills and proficient practical skills.
- Current driver license and ability to travel for client care.
