Health Coach

We are seeking a highly skilled and motivated Senior Health Coach to join our team in Nhulunbuy NT. As a Senior Health Coach, you will play a critical role in delivering health services to clients in the East Arnhem region.

• Assess, plan, implement, and coordinate the Short Term Restorative Care program for clients.
• Work within Community Care Standards and EQuIP Quality Framework.
• Monitor the Health Management of clients over an 8-week program.
• Empower and support clients to achieve their optimal levels of functioning independence and self-care.
• Educate clients, carers, and their families around areas of wellness and reablement, including ongoing chronic disease management.
• Enjoy working independently to promote the service and seek referral of new clients to ensure full occupancy of the program.
• Build and maintain relationships with local health professionals and services to ensure a high quality of care is maintained throughout the program.

• Degree qualification in nursing, allied health, or significant relevant industry experience.
• Registration with relevant professional body or Australian Health Practitioner Regulation Agency (AHPRA).
• 3+ years of clinical or case management experience in Nursing or Allied Health.
• Experience within a health or community aged care setting.
• Ongoing commitment to Continued Professional Development and Education.
• Demonstrated understanding of community aged care guidelines and the principles of primary health care and health promotion.
• Ability to conduct Case Management and Health assessments.
• Strong interpersonal skills, problem-solving, analytical, and reporting skills.
• Ability to work as part of a wider team and autonomously in a remote model of working.
• Well-developed IT skills and demonstrated competence in delivering services via virtual health technologies and a willingness to learn and adopt new technologies.
• Current driver's license and ability to travel within the region on a regular basis.
• Satisfactory police check and current CPR.
