Strategic Support Professional

Your primary function revolves around providing expert support and guidance to participants, their family members, and caregivers in navigating the Scheme and Agency.
Key Responsibilities:- Assess participant needs and develop individualized support plans, making informed decisions regarding treatment, care, and support.
- Provide technical guidance, coaching, and peer supervision to team members, facilitating decision-making and leadership development.
- Conduct peer reviews to ensure high-quality services for participants.
- Identify opportunities to empower participants in developing, implementing, and monitoring support plans that reflect their goals, preferences, and autonomy.
