Job Title: Disability Case Manager

Description:

  • Create and maintain strong relationships between participants and their support teams.
  • Assess participant needs and develop Hearth's support programs.
  • Maintain relationships with support coordinators, house supervisors, and other relevant external stakeholders.
  • Lead the Support Worker teams to provide responsive, person-centered care that enables participants to progress toward their goals.
  • Report on client progress and prepare submissions for NDIS plan reviews as required.
  • Coordinate weekly staff rosters for each participant and resolve rostering issues.
  • Accurately note cases and conduct client reviews that reflect forward planning in line with the participant's NDIS plan.
  • Ensure compliance with policies and procedures to maintain a safe working environment and timely incident reporting.

Requirements:

  • Proven experience working in the disability sector, preferably in a supervisory or managerial role.
  • Professional administrative and business communication skills, including case noting and report writing.
  • Excellent time management and organisational skills.
  • Client-centred approach with the ability to advocate for participants and an understanding of complex disability and trauma.
  • Australian driver's license and reliable vehicle.
  • Strong interpersonal skills and high levels of resilience.
  • Understanding of the NDIS.
  • Working knowledge of Microsoft Office suite and employee rostering systems.
