Job Title: Disability Case Manager
Description:
- Create and maintain strong relationships between participants and their support teams.
- Assess participant needs and develop Hearth's support programs.
- Maintain relationships with support coordinators, house supervisors, and other relevant external stakeholders.
- Lead the Support Worker teams to provide responsive, person-centered care that enables participants to progress toward their goals.
- Report on client progress and prepare submissions for NDIS plan reviews as required.
- Coordinate weekly staff rosters for each participant and resolve rostering issues.
- Accurately note cases and conduct client reviews that reflect forward planning in line with the participant's NDIS plan.
- Ensure compliance with policies and procedures to maintain a safe working environment and timely incident reporting.
Requirements:
- Proven experience working in the disability sector, preferably in a supervisory or managerial role.
- Professional administrative and business communication skills, including case noting and report writing.
- Excellent time management and organisational skills.
- Client-centred approach with the ability to advocate for participants and an understanding of complex disability and trauma.
- Australian driver's license and reliable vehicle.
- Strong interpersonal skills and high levels of resilience.
- Understanding of the NDIS.
- Working knowledge of Microsoft Office suite and employee rostering systems.
